Scope of Services & Non-Medical Treatment

I understand that the services provided by Brilliant Bodywork^® MEDSPA are for cosmetic, relaxation, or wellness purposes only.

Aesthetics: I understand that all aesthetics services or treatments are provided by a a licensed Aesthetician/Cosmetologist in the State of Wisconsin, not a doctor or medical professional.  Services such as surface exfoliations, facial massages, and skin treatments are cosmetic and not medical procedures.

Injectables & Fillers: I understand that all injectable or filler treatments are provided by a licensed nurse injector in the State of Wisconsin, and under the supervision of the medical director for Brilliant Bodywork MEDSPA.

Massage Therapy: I understand that all massage therapies or services are provided by a Licensed Massage Therapist (LMT) in the State of Wisconsin. These services are intended for soft tissue manipulation to improve wellness, circulation and reduce tension.

Non-Diagnosis

I acknowledge that the Brilliant Bodywork MEDSPA does not diagnose illnesses, prescribe medical treatments, or perform spinal manipulations.

No Guarantee of Results

Although all services are FDA-approved, I understand that aesthetic, injectable or fillers, and massage results are not guaranteed. Responses to treatments vary on an individual basis, and I have not received any guarantees regarding the specific outcome of any treatment.

Health & Medical Disclosure

I have fully disclosed to the Brilliant Bodywork MEDSPA all known medical conditions, allergies, medications, and previous complications that may affect my treatment. I agree to inform Brilliant Bodywork MEDSPA of any changes in my health status, including pregnancy, infections, or new skin conditions.

Informed Consent for Treatments

I understand the risks associated with all treatments offered by Brilliant Bodywork MEDSPA, and e.g., chemical peels, microdermabrasion, body treatments, and have had the opportunity to ask questions. I understand I have the right to terminate the treatment at any time.

Limitation of Liability & Indemnification

In consideration for services received, I agree to hold harmless, release, and indemnify Brilliant Bodywork MEDSPA and the individual service providers from any and all claims, demands, or causes of action, known or unknown, arising from any injury, damage, or condition that may arise from my treatment, provided the provider acted within the scope of their license.

Confidentiality & Privacy Policy

I understand that Brilliant Bodywork MEDSPA will keep my personal information confidential, as required by Wisconsin and all applicable laws or regulations, unless disclosure is authorized by me or required by law. More information can be found in our website Privacy Policy.
